I am making the following FOIA requests:

1. Please provide me copies of documents that list by name all baseball coaches
and assistant coaches for the 2006 season and the upcoming 2007 season.
2. Please provide me copies of documents that attest to the fact that the above
coaches and assistant coaches have received the required VHSL training.
3. Please provide me copies of documents that list the above coaches who have not
received this training and who are coaching.

Please let me know the cost of this request if any.

I am making the following FOIA request regarding the recent survey that was performed at Lake Braddock high school about their sports programs and to be more specific their baseball program and coaches.  Please provide me copies of all documents relating to this survey. Instructions on how to complete this survey, blank forms, completed forms and any summary of findings.
